Are you (or are you a Carer for) someone who has been diagnosed with Dementia/ Alzheimers and are due to attend a Hull Hospital appointment (or attended an appointment recently)?
Alzheimers Society have a new project that aims to help provide information and support for individuals in these situations and want to hear from the following people:
Hull or East Riding residents who have been diagnosed with Dementia/Alzheimers and are due to attend a Hull Hospital appointment this year (or their Carers/ family members) and are concerned/ seeking information or support.
Hull or East Riding residents who have been diagnosed with Dementia/Alzheimers and have recently attended a Hull Hospital appointment in 2020. (or their Carers/ family members) That are concerned/ seeking information or support about their diagnosis or other issues/ information about Dementia/ Alzheimers.
Charities/ Community Groups/ Health Care Workers/ Self Help Groups who could consider referring people diagnosed with Dementia/ Alzheimers who they are aware of who may benefit from services (with the persons consent for the referral)
The aim is to help people to deal with the emotions and concerns of attending hospital and/ or afterwards and help to find the information and support available from service providers and others.
