Community Services Information Event

City Health Care Partnership (CHCP) have recently been commissioned to deliver community services across Hull and the East Riding.
Community Services cover a wide range of health and social care activity including:

• Community nursing
• Community therapies
• Planned care
• Anticipatory care / frailty
• Urgent / unplanned care
• Sexual health
• Children and Young Peoples Services

The aims of the recommissioning is to move towards more equitable community services, based on local need, to develop more streamlined and effective pathways of care and a more integrated approach to service delivery.
As part of the commissioning process CHCP are reviewing these services and have been working with stakeholders to improve the process.
