Community Connectors (x4) with Mind HEY

37 Hours per week based across the East Riding

£15,261 per annum

These are new posts to work in partnership with NHS Community Link Workers on the Social Prescribing contract.

Using your knowledge of local service provision, health needs and issues across the East Riding you will empower, motivate and support individuals to access community resources to improve their health and wellbeing.

Highly developed interpersonal skills will be key to the success of this role, you should also be competent in the use of IT systems.

Applicants must have a full current driving licence and access to a motor vehicle for work purposes as travel will be required.

The service will be delivered in the localities of:   Goole/Howden; Bridlington; Beverley/Driffield; Pocklington/Market Weighton and Holderness

They would particularly welcome applicants who live in these areas.

These posts are subject to an Enhanced DBS Disclosure.
