Funding: Connect Well Social Prescribing Grants Round 3: Support for Housebound People

Many people may be unable to leave their homes at all, or without substantial assistance from others, leaving them isolated and excluded and vulnerable to loneliness. People may be entirely housebound due to illness; disability; frailty due to advancing age; or mental health issues such as agoraphobia, social phobia, anxiety and hoarding for example. Or they may have extremely limited capacity to get out of their homes unaided, due to caring responsibilities, mobility and sensory limitations, or language/cultural limitations.
 
This third round of Connect Well Hull grants seeks to support projects and services that reach out to people who are housebound. Eligible applications will be for work – either new, or that expands the capacity of existing work – that:
 

• Supports people aged 16 and over who live in Hull or are registered with a Hull GP
• Reduces isolation and loneliness, and improves wellbeing of people who are housebound (e.g. through befriending)
• Increases access to services and facilities for those who are housebound
• Increases independence of people who are limited in their ability to leave their homes without substantial assistance

Background
Connect Well Hull is the new ‘social prescribing’ service for people who live in Hull or are registered with a GP (doctor) in Hull. The service is delivered in partnership by Citizens Advice Hull & East Riding and North Bank Forum. It helps people to access support and guidance on a range of issues, including linking people with community services and advice on money, benefits or housing problems.
The programme was commissioned by NHS Hull Clinical Commissioning Group (CCG) and Hull City Council. In addition to providing advice and support, the service will help to identify gaps in services across the city.
 
To help address these gaps, commissioners are making a further investment in health and wellbeing outcomes through a targeted grant fund over three years.

A briefing and networking session will take place in August, providing potential applicants with an opportunity to find out more about the fund, the Connect Well service and how to apply as well as a chance to network with other potential applicants to discuss gaps and ideas. Applications will be assessed by a panel that includes representatives from NHS Hull CCG and Hull City Council. 

Deadline for applications is 9am Monday 17^th September 2018 applications received after that time will not be accepted.
