Hull: 2 x Funded Intensive Rough Sleeper Outreach Worker Posts, Emmaus

This role is responsible for delivering and coordinating delivery of a Rough Sleeper Support Service in Hull, targeting entrenched rough sleepers and focusing on proactive support for an identified caseload, assisting direct access provision options, health, all appointments with supporting agencies.

Designed to work alongside the existing Emmaus Outreach Service and homelessness network across Hull & East Riding this role is pivotal in linking the most marginalised rough sleepers into suitable accommodation. This role involves supporting Emmaus Companions to become effective Outreach Support Workers, and ensuring that progression is supported and monitored closely for the best possible outcomes. The role includes offering intensive one and one support, multiagency coordination and reporting.
