Hull City Council are looking to recruit an experienced and professional full time Early Help Family Support Worker to support families of children with Special Educational Needs and Disabilities (SEND).
The successful candidate will provide early, targeted support for families with children and young people (0-19 years) with (SEND) across the city. This support will be part of an integrated approach to prevent family problems from escalating and to bring about sustainable change, increasing the resilience of the family and improving outcomes.
They will be expected to develop and deliver a range of targeted Early Help services, as part of an integrated Early Help Hub, and will be a Lead Practitioner for families with children and young people with a range of SEND needs, undertaking assessments and co-ordinating a response to the needs of families in partnership with other agencies.
