Hull: Social Prescribing Link Worker vacancy with Forum

Forum are seeking a skilled Social Prescribing Link Worker to work across GP practices in Hull on behalf on one of Hull’s Primary Care Networks.

The successful candidate will be employed by Forum to offer ‘social prescribing’ support to patients in Hull. This role provides support to individual users of the Primary Care Social Prescribing service and their families and carers and refers them into a wide range of voluntary and community services and activities.  This role requires significant communication skills and experience of communicating with a diverse range of people.

Salary £20,867pa; Full time 37hours pw

28 days leave plus statutory holidays
