Vacancy: Link workers – Social Prescribing (Hull)

Forum is recruiting for 2 x FTE Link Workers to work across GP practices in Hull, on behalf of two of the city’s Primary Care Networks. The successful candidates will be employed by Forum to offer ‘social prescribing’ support to patients in Hull, referring them into a wide range of voluntary and community services and activities.
