Health Captains programme is a pilot initiative to identify ways that volunteering initiatives can help reduce pressures on NHS services and empower people in their health and care journey.
Since March 2024 Hull CVS have been delivering the Long-terms Conditions Buddy service, offering regular telephone support for those who have been discharged from hospital and/or have a long-term health condition. The support is delivered by Hull CVS’s volunteer team and offers clients a listening ear and reassurance.
The team are keen to link up with local organisations, services, and communities to:
• Raise awareness of health inequalities and early intervention/prevention in local areas. For example, smoking cessation and early cancer diagnosis
• Promote the Health Captains programme, specifically the Long-term Conditions Buddy (LTCB) Service. To reach out to those who may benefit from receiving LTCB support from volunteer buddies in Hull and East Riding.
• Reach out to those interested in volunteering. Our wonderful and caring volunteers at Health Captains are what keep our service going and help so many people across Hull and East Riding.
We are looking for caring and empathic people with experience of living with a long-term health condition or caring for those who have a long-term health condition, who are interested in volunteering. We know life can be busy, our volunteering roles are flexible, ‘micro- volunteering’ roles and can be done as and when/around the volunteers’ lifestyle.
