Home from Hospital are a local service who aim to provide a high quality short-term service to adults during the vulnerable period on discharge from hospital, who reside within the boundaries of Kingston Upon Hull.
They provide reassurance, practical support and assistance in rehabilitation for those returning home alone or with little support networks locally. Home from Hospital encourage and promote the return to self volition, independence and self confidence through innovative and flexible service provision
Home From Hospital Service originated, from a research audit and needs analysis undertaken in 1993/94. The community audit was completed by 5000 households and informed the structure and content of the project – i.e. free short-term 6-8 week support for older people on discharge from hospital: The aim to assist vulnerable people through a period of convalescence to continue confidently in their own homes.
They are a volunteer led organisation and their volunteers who are members of the local community support their neighbours to regain and maintain their independence, allowing them to be in control of their lives.
All services and projects are maintained through constant recruitment and training of volunteers. All volunteers are DBS checked, trained in health and social care skills, and are fully insured. Many move on to full employment or higher education and they receive an excellent quality of education and support from qualified staff.
